Services Clinical Associates of Tidewater is a multispecialty private practice consisting of Clinical Psychologists, and Social Workers. We have offered comprehensive psychological health care services to Peninsula residents since 1983. We offer a wide range of diagnostic and psychotherapeutic services, which include: psychological testing, with intellectual, educational and personality inventories; consultation and psychotherapy with children, adolescents, adults, couples, and families; and consultation services with pediatricians, family physicians, schools, courts, and social service agencies. New Patient Appointments. When you arrive for your first appointment, you will be asked to complete a new patient information form. Some of the information on this form duplicates several questions asked during your initial telephone contact. Please take time to complete this form as it becomes an important part of your records. Appointment Scheduling. We operate on an appointment only basis. Because of this we ask that you make contact with the office 24 hours in advance to cancel a scheduled appointment so that the time can be used by someone else. If cancellation is not received in advance, a charge will be made. Missed appointment fees are not covered by insurance. Confidentiality Information about your treatment remains confidential. No information is released without written permission, within the limits prescribed by state law. We may, from time to time, request permission to release or obtain information for specific purposes. Do not hesitate to raise a question if you do not understand the reason for such a request. Financial Policy We prefer direct payment in full for services rendered, either at the time of service or on a monthly basis. Alternatively, we will accept assignment of insurance benefits to us, in which case you will pay only deductibles and copayments directly to us at the time of your appointment. Our office staff will be glad to discuss with you in detail the alternative payment plans available. Insurance When you pay directly in full for services rendered, we expect that you will file your own insurance claim. We will provide you with all the information you will need for reimbursement. When we arrange for assignment of benefits, and your insurance carrier is to pay us directly, then we will file the insurance claim. Whichever method we use, we will look to you for final payment of your balance. Office Hours 9:00 AM - 7:00 PM Monday through Thursday 9:00 AM - 4:00 PM Friday Alternative office hours are available by arrangement with individual providers. When the offices are closed, do not hesitate to call if the need arises. Office telephones are covered by an answering service at those times, and messages will be relayed to your provider. Emergencies In the event of an emergency, you should call the office telephone number. Either the office staff or the answering service will see that your call is relayed to your provider immediately. If for any reason your provider is not available, another member of the professional staff will respond to your call. In a severe emergency please go to the nearest hospital emergency room and ask to see a psychiatrist, who will contact your provider. |
